Some men complaint of shortened penis following prostate cancer treatment

Jan. 2, 2013 ? A small percentage of men in a prostate cancer study complained that their penis seemed shorter following treatment, with some saying that it interfered with intimate relationships and caused them to regret the type of treatment they chose.

Complaints were more common in men treated with radical prostatectomy (surgical removal of the prostate) or male hormone-blocking drugs combined with radiation therapy, according to the study by researchers from Dana-Farber/Brigham and Women's Cancer Center (DF/BWCC). No men reported a perceived shortening of their penis following radiation therapy alone.

The study's findings, which are being published in the January issue of the journal Urology, are based on surveys completed by physicians of 948 men treated for prostate cancer and who had suffered a recurrence of the disease.

Twenty-five men (2.63 percent of the group) complained of smaller penises after treatment -- 3.73 percent for surgery, 2.67 percent for radiotherapy plus androgen deprivation therapy (ADT), and 0% for radiotherapy alone. Radiotherapy included both radiation administered by an external x-ray machine, and brachytherapy -- the implantation of radioactive seeds directly into the prostate.

The scientific team, led by Paul Nguyen, MD, a radiation oncologist, and medical student Arti Parekh, said it is the first study to link men's perceptions of a reduction in penis size to lowered life satisfaction, problems in emotional relationships, and misgivings about the specific form of prostate cancer treatment they chose.

Nguyen said that the potential side effect of a smaller penis is well-known among physicians and surgeons, said Nguyen, "but it's almost never discussed with patients, so it can be very upsetting to some men when it occurs. Patients can deal with almost any side effect if they have some inkling ahead of time that they may happen."

The report's authors said physicians should discuss the possibility with their patients so that they can make more-informed treatment choices.

There were no direct measurements of penis size either before or after treatment, said the researchers. Nor did the patients' physicians specifically ask about this side effect; the issue was brought up by patients in conversations with their doctors. For this and other reasons, the authors of the new study suggest that the problem is likely more common than reported in the survey.

"Prostate cancer is one of the few cancers where patients have a choice of therapies, and because of the range of possible side effects, it can be a tough choice," said Nguyen. "This study says that when penile shortening does occur, it really does affect patients and their quality of life. It's something we should be discussing up front so that it will help reduce treatment regrets."

The likelihood and magnitude of penis shortening as a consequence of treatment have not been well studied, said the researchers. However, Jim Hu, MD, a surgeon at the University of California, Los Angeles Medical Center and a co-author of the study, said "Previous studies have concluded that there is shortened penis length following prostatectomy. This is most common with non-nerve sparing surgery, as this may result in fibrosis and atrophy of erectile tissue due to damage to nerve and vascular structures." The present study did not find much difference on that score.

The study's subjects were men enrolled in a registry called COMPARE that collects data on patients whose prostate cancer shows signs of recurring after initial treatment. Of the 948 men in the study, 22 percent were younger than 60 and the majority were in their 60s, 70s and 80s. Just over half -- 54 percent -- had undergone surgery to remove their cancerous prostate, while 24 percent received radiation therapy combined with hormone-blocking treatment, and 22 percent had radiation therapy alone.

In an editorial comment accompanying the report, Luc Cormier, MD, PhD, of Dijon University Hospital in France said the study "is really of interest because of the number of patients and that it included other treatment methods in addition to radical prostatectomy."

The surveys of the men did not report on their sexual functioning. Cormier observed that "sexual activity needs to be thoroughly measured owing to the obvious relationship with the patients' perception of penile length."

Year in Review: Arts and Entertainment - Summerville Journal Scene

By Jim Tatum
Summerville Journal Scene ?

Chelsea Summers, shown here in July performance at Coastal Coffee Roasters was quite busy in 2012, playing at a number of venues locally and out of town.

Summerville and Dorchester County saw a variety of fun and interesting performances, shows, and exhibits throughout 2012. From old pros to new faces, the area continues to be a vibrant and exciting place to feed the artistic soul.

In January, a young Summerville singer took a big step toward her future. Thirteen-year-old singer Shayla Degregorio, known in musical circles as ?Shayla D,? was one of 18 young artists from North and South Carolina featured on a compilation CD called ?Pride of the Carolinas,? which featured up and coming new talents from the Carolinas.

But Shayla was not the only one to get something of a career break this year.

Chelsea Summers, a talented, up and coming singer/songwriter/guitarist opened for the nationally known rock band Parachute Saturday at Ziggy?s Main Music Hall in Winston Salem, N.C.? Chelsea would stay busy throughout 2012, playing gigs in a number of local venues including Coastal Coffee Roasters, Icehouse Restaurant, Single Smile Caf?, Firewater Grille and Madra Rua Irish Pub. She was a featured performer at the Flowertown Festival and also opened for national recording artist Justin James a special performance in Coastal Coffee Roasters in July.

Of course, singer/songwriter/guitarist extraordinaire Eddie Bush stayed busy, not only playing local and regional events such as Flowertown Festival but traveling nationally with with his band, known as Eddie Bush and the Mayhem. Bush also released a new CD, ?Calloused Hands? in December.

Another local musician who stayed busy but often flies under the radar was Jesse Frank, a singer/songwriter/musician who also owns and operates Lowcountry Music Academy on West Richardson Avenue in Summerville. Frank was busy with several projects, including putting together a new band, Flowertown Theory, as well as writing and recording in the studio. He also entered one of his songs, ?Stolen Lullaby? in the John Lennon Songwriting Contest, an international event open to amateur and professional songwriters.

Possibly following in the footsteps of these accomplished performers were a unique group of young people, students from Herve Moreau?s Music Exploratory and Music Technology classes at Pinewood Preparatory School. For the second year in a row, Moreau?s students went head to head in a songwriting contest, held in Pinewood?s Liberty Garden in March. The songwriters ranged in age and experience from 6th graders to high school seniors. The winners of the contest received free studio time to record two of their songs.

The classes also hosted a fundraising event in April known around the school as ?Pinewoodstock,? which showcased Pinewood musicians in an outdoor venue setting on the school grounds.

Pinewood Prep also held its third annual Pickin? in the Pines Bluegrass and Barbeque Festival in November. The event included live performances by several area bluegrass bands, including Homegrown, which features two Pinewood Students.

On the visual and performing arts side of things, the Summerville Artist Guild held their 36th Annual Judged Show with a reception at the?Town Hall during the spring.? The show Karen Ann Myers, Assistant Director of The Halsey Institute at the College of Charleston, judged the show, which hung in Town Hall until the end of March. Later, the guild would feature their annual photography show, which was on display in Town Hall until December.

Meanwhile, the Firewater Grille in Summerville successfully hosted comedy shows on Friday and Saturday nights throughout 2012. Not only have the shows been well received, their popularity has grown and continues to grow, says owner Bob Shipley and comedy producer Tony Kemp.

In fact, Summerville has become quite the popular underground gig for a lot of A-List comics these days, including such talents as Darryl Rhoads, Peter Hefty and Justin, Janet ?The Tennessee Tramp? Williams, and Shaun Jones.

A relative ?new kid on the block? entered the scene in August as Art and Soul opened in Summerville in August. Art and Soul hit the ground running, sending mimes out on opening night to guide Third Thursday patrons to the new space. In November Art and Soul hosted a special fundraiser for the Friends of the Dorchester County Library which featured a showing of works from the private collection of Robert Seay, a long-time private art collector who lives in Summerville. The objects on display included works of old masters from between the 15th and 20th centuries, including Pablo Picasso, Salvador Dali, William Hogarth, and Albrecht D?rer.

The Summerville Community Orchestra continued to stay busy with a number of performances throughout the year featuring exciting guest performers and wide variety of musical selections. No doubt, SCO?s 2013 season will be just as busy and well attended.

Palestinians will outnumber Israeli Jews by 2020

JERUSALEM (AP) ? The Palestinian statistics bureau estimates that Arabs will outnumber Jews in the Holy Land by the end of the decade, a scenario that could have grave implications for Israel.

The bureau said Tuesday that 5.8 million Arabs live in Israel, the West Bank, Gaza and east Jerusalem. That compares to about 6 million Israeli Jews, according to Israeli data.

It said that based on current birth rates, the two populations would be equal in 2016, and in 2020, Arabs would outnumber Jews by 7.2 million to 6.9 million.

The demographic issue is a main argument for Israeli backers of creation of a Palestinian state. They say relinquishing control of the Palestinian territories and its residents is the only way to ensure Israel's future as a democracy with a Jewish majority.

Control what you can - Business Management Daily

Good news for employers that try their best to maintain a harassment-free workplace, but sometimes fail: Courts understand there are limits to what employers can do when it comes to limiting all racially hostile comments and acts.

Recent case: Jacqueline, who is black, was a nurse at a community hospital. She protested when she was assigned to work with a patient who was known for making ?intolerable racist comments? and for targeting ?minorities within the staff and making complaints? against them. After the patient, who had dementia, ?repeatedly denigrated? Jacqueline and called her the N-word, she de??manded that management stop the abuse.

Supervisors explained that they had tried, but couldn?t do anything to prevent the patient?s outbursts. They told her that it was something she would ?just have to deal with.?

Jacqueline sued, alleging she had been forced to work in a racially ?hostile work environment.

The court dismissed her case after the hospital explained that there wasn?t anything it could do to make an Alzheimer?s patient be??have the way Jacqueline wanted. It concluded the hospital wasn?t responsible for the abuse because there really wasn?t a practical way to end it. (Wright v. Monroe Community Hospital, No. 11-3520, 2nd Cir., 2012)

Final note: Control what you can, don?t worry about what you can?t. You can punish co-workers, supervisors, prisoners, students and other individuals over whom you have control and influence for aberrant behavior. For example, if a customer is abusive, you can and must stop it, even if it means losing the customer.

On the other hand, there isn?t much you can do to stop name calling by those with mental disorders, especially in an institutional or health care setting where you must provide services to all.

The Causes and Cure of Hemorrhoids - Health Fitness ...

The Causes and Cure of Hemorrhoids ? Health Fitness. Most people of adult age have probably suffered the indignity of having to cope with Hemorrhoids, otherwise known as Piles. As anyone who has suffered from piles knows, these can be very uncomfortable and painful. What exactly are Hemorrhoids? In a medical sense they are enlarged or swollen veins. They are somewhat similar to varicose veins that develop in the legs, except that these are veins located in the anus. These veins cushion and protect the anal canal. Problems arise when these veins are stretched and tear under pressure, resulting in the familiar discomfort and pain experienced with haemorrhoids. One may experience hemorroids as either internal or external. Internal hemorrhoids are located higher up in the anus and normally manifest themselves as stools covered in blood or blood on the toilet paper. Sometimes the hemorrhoids will protrude from the anal canal, in which case they should just be pushed back inside. External haemorrhoids, on the other hand, exist as hard lumps that can be felt by hand and are extremely painful compared to internal haemorrhoids. These may also cause bleeding which will be seen on the toilet paper or in the toilet bowl. Causes of Piles Constipation tends to be the main cause of Piles. The straining of bowel movement during prolonged siting on the toilet puts pressure on the blood vessel in the anal canal, causing swelling and irritation and eventual rupture of the blood vessels ?
